First national survival data for colorectal cancer among Saudis ...
??First national survival data for colorectal cancer among Saudis between 1994 and 2004: what's next?
Colorectal cancer (CRC) is the second most common malignancy in the Saudi population. This study aimed to review CRC data from the Saudi Cancer Registry (SCR) in order to evaluate the prognostic factors for CRC survival in Saudi patients.Methods: This study was a retrospective censored overall survival (OS) analysis of CRC data for the period 1994--2004 obtained from the SCR.
Data were collected from all 13 administrative regions of the Kingdom of Saudi Arabia (KSA) by the SCR in collaboration with the National Information Center of the Ministry of Interior. The Kaplan-Meier method was used to calculate the cumulative survival rate, which was then stratified by gender and by period (1994--1999 versus 2000--2004).
The clinico-pathological variables that might affect CRC survival were analyzed by Cox regression analysis.
Results: Between 1994 and 2004, 549 CRC cases were diagnosed (363 [66.1%] in males and 186 [33.9%] in females). The OS for CRC during this period was 44.6% (44.7% for 1994--1999 and 44.3% for 2000--2004 [p=0.7]).
There was a significant (p=0.003) discrepancy of 9.6% between the male five-year OS (41.0%) and the female five-year OS (50.6%). The five-year OS was 63.3% for patients with localized disease, 50.2% for those with regional disease, and 14.7% for patients with metastases.
By Cox regression analysis, age and extent were significant prognostic factors of survival in patients with colon cancer; the risk was higher in patients with distant metastasis (hazard ratio [HR], 2.53; 95% confidence interval [CI], 1.17-5.45; p=0.01). In patients with rectal cancer, the risk was lower in males (HR, 0.66; CI, 0.45-0.98; p=0.04), but higher in patients with unknown tumor extent (HR, 3.70; 95% CI, 1.66-8.24; p=0.01).
Conclusions: The five-year OS for 1994--2004 was 44.6% for patients with CRC.
More so, five-year OS based on CRC stage was generally lower than the typically reported survival rates. The establishment of a national screening program and increased access to specialized medical faculties may be necessary to improve CRC survival in the KSA.

WASHINGTON (AP) ? From gun control to gay rights, President Barack Obama's second-term agenda is shaping up as an unabashedly liberal wish list.
In less than a week, he's vowed to tackle climate change and protect government entitlements. His administration has lifted a ban on women in combat and expanded opportunities for disabled students. Proposals for stricter gun laws have already been unveiled, and plans for comprehensive immigration reform, including a pathway to citizenship for millions of illegal immigrants, are coming next week.
Obama's full embrace of such an agenda suggests a president both freed for action by his re-election win and seeking to capitalize on it.
"There is a deep recognition that he has a short period of time to get a lot done," said Jennifer Psaki, Obama's 2012 campaign spokeswoman. "The American people are seeing a man who is determined to finish what he started in his first term, pushing through his agenda without the burden of running for re-election."
But following through and winning approval for his proposals will require cooperation from a Congress that is nearly as divided now as it was before the November elections.
"If the president pursues that kind of agenda, obviously it's not designed to bring us together," said Senate Minority Leader Mitch McConnell of Kentucky, who calls the start of Obama's second term "a new era of liberalism."
And it's not just congressional Republicans who could stand in Obama's way as he seeks to make good on his pledges. Senate Democrats from conservative-leaning states ? who, unlike Obama, still face future elections ? may have reservations about backing a liberal agenda in the lead-up to the 2014 midterms.
Democratic resistance is already proving to be a problem for some of the toughest gun control measures that Obama proposed ahead of his inauguration in response to the elementary school shooting in Newtown, Conn. An assault weapons ban, in particular, appears to be in jeopardy, with Senate Majority Leader Harry Reid of Nevada among the Democrats yet to voice support.
Obama's tilt to the left follows a presidential campaign that left open questions about what policies he would pursue if he won a second term. His most specific campaign pledge was to let George W. Bush-era tax cuts expire for the wealthiest Americans, a step he was able to achieve during the "fiscal cliff" negotiations in late December.
That's why many of his strongest supporters were surprised ? some pleasantly ? by the issues he raised in his inaugural address, particularly his call for tackling the threat of climate change, a topic that garnered little attention during the campaign.
And after campaigning on a balanced approach to deficit reduction, including making tough choices on entitlement programs, Obama used his inauguration to extol the virtues of Medicare, Medicaid and Social Security. He made just one reference to the huge federal deficit.
Obama has previously said he's willing to put government entitlements on the table as Washington seeks to reduce the deficit, and aides say his inaugural address doesn't change that.
Obama also became the first president to use the word "gay" in an inaugural address, asserting that the nation's journey is not complete until "our gay brothers and sisters are treated like anyone else under the law."
Progressive groups welcomed the president's rhetoric, but made it clear that words alone would not be enough.
Adam Green, co-founder of the political action group Progressive Change Campaign Committee, said that if the president's inaugural address does become a template for his second-term governing strategy, "that will allow the president to win big victories and secure a legacy of bold progressive change that helped millions of Americans."
"We hope he goes that route, and we will proudly rally to his side if he does," Green said.
It remains to be seen how vigorously Obama pursues the agenda items he outlined in his inaugural address. White House officials have already struggled this week to back up his rhetoric with specific policy proposals
Still, people close to the president say he feels emboldened since winning re-election and wants to act quickly on progressive issues that eluded him during his first term. His supporters have also created a campaign spin-off organization to support his second-term agenda.
The outside group, Organizing for Action, will employ many of the same people who led Obama's two presidential bids and will have access to his campaign's voter and donor data. The new group is expected to focus on backing his positions in legislative battles, including gun control and immigration.
Some of what the president may pursue, particularly when it comes to climate change, may be through executive branch actions that don't need congressional approval.
The Pentagon this week acted on its own to lift a ban that kept female service members out of combat positions. And the Education Department unilaterally ordered schools to include students with disabilities in sports programs or provide equal alternative options.
White House officials are scouring rule books for other actions the West Wing and government agencies can take on their own.

Use the Socratic Method to Easily Win Arguments
Arguments become heated and get out of hand when anger enters into the equation, often leaving both parties frustrated and upset. Using Socratic questioning in an argument, however, can help people see things from your perspective without causing unnecessary conflict.
Rather than making points, the Socratic method concentrates on asking questions to help both parties think about the situation rather than just verbalize their (often exaggerated) feelings. Redditor darkcity2 explains:
[L]et's say your girlfriend is angry about you coming home late one night. She says, "You didn't call me! I was so worried, and you were probably out at the bar hitting on girls!" You could respond, "No baby, I wasn't, I swear." But effectively this is two heads butting; the one with greater force wins. Instead, ask questions. This puts you in a position of control without being aggressive, and forces the listener to accept their own hypocrisy/flawed reasoning. Say something like: "Why are you upset? What do you think I was doing? Is it that I'm late that bothers you, or that you can't trust me?" Rather than arguing back and forth, you're encouraging her to express how she feels, and it seems like you're making an effort to listen. Eventually she'll either forgive you, or you'll see that you were wrong, or she'll see that she was being unreasonable.
I gave a very sexist example, but this works for heated debates as well. Rather than saying, Animals aren't meant to be in captivity, say, Do you think animals were meant to be in captivity? Wording your point of view in the form of a question forces them to see it from your perspective whether they like it or not, because a question psychologically puts them in a position where they have to answer it.
It's a pretty simple idea and an easy conflict resolution tactic. Next time you find yourself in an argument, give the Socratic method a shot.

French encircle key Malian town of Diabaly
BAMAKO, Mali (AP) ? French troops encircled a key Malian town on Friday to stop radical Islamists from striking closer to the capital, a French official said.
The move to surround Diabaly came as French and Malian authorities said they had retaken Konna, the central city whose capture prompted the French military intervention last week.
The United Nations warned that some 700,000 civilians could be displaced by the fighting in Mali, where the French-led international force is fighting to oust the rebels from power in the north.
The French forces moved around Diabaly to cut off supplies to the Islamist extremists, who have held the town since Monday, said a French official who spoke on condition of anonymity to be able to discuss sensitive security matters.
The Malian military chased the Islamists from Konna and are now holding the town, a Malian military official, who insisted on anonymity because he was not authorized to speak to journalists, said Friday.
Telephone lines were cut off in the town, making it difficult to independently verify the claim though the French official also confirmed the rebels no longer hold Konna.
Doctors Without Borders has been trying to get to Konna since Monday but all roads leading to the community in central Mali have been closed by the Malian military, said Malik Allaouna, director of operations for the group known as MSF for its initials in French.
"Despite our repeated requests, we are still being refused access to the Konna region," he said. "It is essential to allow the delivery of neutral and impartial medical and humanitarian aid in the areas affected by the conflict."
In Geneva, United Nations refugee agency spokeswoman Melissa Fleming said that the number of displaced Malians is expected to increase dramatically in the coming months.
Those who have fled "mentioned that large amounts of money are being offered to civilians to fight against the Malian army and its supporters," she said.
Meanwhile, authorities in the town of Niono said that a key road leading to Segou was being closed to traffic Friday between 6 p.m. and 8 a.m.
Segou is one of the largest towns in Mali and the administrative capital of its central region. The road closure is an attempt to keep Islamists from entering towns further south, officials said.
The prefect, or district administrator, of Niono Seydou Traore said "neither cars, nor motorcycles, nor people on foot will be able to travel, as a security measure."
France has encountered fierce resistance from the extremist groups, whose tentacles extend not only over a territory the size of Afghanistan in Mali, but also another 600 miles (1,000 kilometers) to the northeast in Algeria, where fighters stormed a BP-operated plant and took dozens of foreigners hostages, including Americans.
They demanded the immediate end of the hostilities in Mali, with one commander, Oumar Ould Hamaha, saying that they are now "globalizing the conflict" in revenge for the military assault on Malian soil.
On Thursday, France increased its troop strength in Mali to 1,400, said French Defense Minister Jean-Yves Le Drian.
West African neighbors also have begun sending troops to aid the French-led mission, with some Togolese and Nigerian forces arriving Thursday.
Nigeria has offered another 900 soldiers, while Chad has said it will send 2,000 to aid the mission.
A former French colony, Mali once enjoyed a reputation as one of West Africa's most stable democracies with the majority of its 15.8 million people practicing a moderate form of Islam. That changed last March, following a coup in the capital which created the disarray that allowed Islamist extremists to take over the main cities in the distant north.

Springing into the new year, internet radio heavyweight Pandora has released some of its major statistics from 2012. The company states that 1.6 billion personalized radio stations were created and that it played over 13 billion hours of music. These hefty stats translate to the service's users listening to over one million different songs from more than 100,000 artists. Wrapping up last year's scorecard, around 10,000 artists reached about 250,000 unique listeners. While these numbers are impressive, we can't help but wonder how many audio commercials had to be endured so people could continue listening to countless hours of One Direction. In any case, we're sure it was worth it.

Microsoft Office 2013 now available to business users via Home Use Program
Microsoft is continuing to get all its Office 2013 ducks in a row as it approaches the still-unconfirmed but imminent launch date for its newest Office release.
The latest piece that's fallen into place is the addition of Office Professional Plus 2013 to the company's Home Use Program (HUP). (Thanks to Neowin for the tip.)
HUP is a benefit that Microsoft offers to volume licensees with Software Assurance. It allows business users to buy and use the latest version of Office to use on their home PCs/devices through a "low-cost" download. (Neowin says the Professional Plus 2013 SKU will cost about $9.95 per user via HUP.)
Microsoft typically makes available latest version of Office to HUP around the time that a new version of Office becomes generally/commercially available. Microsoft officials still have not revealed when Microsoft plans to launch the new Office, but it's expected some time in the next few weeks, and possibly by the end of this month.
The Professional Plus version of Office 2013 is available only to volume licensees. This particular SKU includes the 2013/newest versions of Word, Excel, PowerPoint, OneNote, Outlook (with Business Contact Manager), Publisher, Access, InfoPath, Lync and the Office Web Apps. Professional Plus is one of a handful of new Office 2013 SKUs that Microsoft will be launching.
In other Office-related news, those who've been testing the preview version of Office 2013/Office 365 Home Premium shouldn't worry that their test versions will expire before Microsoft launches the final version of the product.
A Microsoft spokesperson told me that those seeing expiration notices in the billing section for the Office 365 Customer Preview trials?should know that the preview actually won't expire until approximately 60 days after the next version of Office becomes available in their market. Preview versions obtained via Office 365 will auto-renew, with newer/later dates displayed when users check back into http://commerce.microsoft.com.
The notification dates were built into the preview version to be in-line with how subscription users will be notified that an Office 365 subscription or trial will expire and remind them to renew or purchase once the next version of Office is available in their market.
"The messages trial customers see now are essentially of the Customer Preview experience as the team tests the system," the Microsoft spokesperson said. "Trials continue to live on without people having to do anything."

No vote on American Dream Meadowlands at sports authority meeting
EAST RUTHERFORD ? The revised master plan for the American Dream Meadowlands project will have to wait at least another month.
The plan was not on the agenda of the New Jersey Sports and Exposition Authority board's monthly meeting Thursday, and the board did not discuss the American Dream.
The revised master plan reflects the addition of an amusement park and a water park to the project. Triple Five, the developer of American Dream, has been waiting for the sports authority's approval since the Environmental Protection Agency gave the project its blessing in August.
Wayne Hasenbalg, president and CEO of the sports authority, has kept quiet about when a vote will come.
Many think it will be soon, as optimism around the project increases. Jon F. Hanson, who is negotiating a financing deal between Triple Five and the state, hinted that construction could start in January.
But the sports authority's approval may bring new delays. While a judge dismissed a lawsuit by the Jets and Giants to stop the project over the teams' objections to the amusement and water parks, he said the teams may sue again when the revised master plan is approved.

We always love a big kitchen renovation when it comes to Reader Redesigns, but sometimes making over just the corner is enough to get us inspired bigtime. Just check out what Deme and her family did to the breakfast nook of their dated kitchen. Here?s her email:
Hi Young House Lovers! Your blogs is one of the highlights of my day and I wanted to share our? breakfast nook with you. After removing a thick lacquered chair rail and scrapping layers of fruit patterned and 70?s wallpaper, we ended up with some pretty fugly untextured walls.
We started with a dark dungeon of a corner and ended up with something bright, fresh and uber functional for our young family of five!
I scored an L-shaped bench on craigslist to give us some additional seating (our previous table could seat four, leaving someone in our family out of luck at mealtime). We said goodbye to the honey finish, giving it a fresh coat of white paint.
Even though the benches were a pretty standard style, they had storage in the seats, which helped solve the problem of our art/craft supplies that were scattered throughout the house. We lined them with some graphic shelf paper and they now hold all of the kids? painting/art supplies, homework necessities, and learning activities for our youngest bambinos.
When it came time to paint the walls, I planned to use stripes to help this corner feel larger. But I also can?t help but be pulled toward the ombre trend, so we went with a subtle ombre stripe treatment for our backdrop.
We used a digital scrap booking kit to break up one of our favorite Dr Seuss quotes into four prints (?The more that you read, the more things you will know. The more that you learn, the more places you?ll go.?). We framed them with some white lovelies we grabbed on sale at Target? they mark this space as a place to learn, explore, and create!
Overall we are so happy with the change to this little kitchen corner! It serves so many functions for us and I can?t help but smile whenever I walk into the kitchen? especially since the rest of the kitchen is in that ugly mid-cabinet-painting stage of DIY kitchen reno! ~Deme
